SCPS Professional Certificate Request Form

SCPS Professional Certificate Completion:

  • You must complete the Certificate Program within four years.

    Within one year of completion of course requirements, you must request your certificate by mail, fax, or online.

  • For information concerning academic policies related to SCPS professional certificates consult the continuing education academic policies and procedures area.
    Some departments may establish more stringent requirements for selected programs. Check individual listings for details.

  • Please make sure all your grades have been posted before requesting your certificate. At the end of each term, you can check your grades from your account, or obtain grades by sending a request via fax to (212) 995-4154 or by mail to: Office of the University Registrar, Transcripts Department, P.O. Box 910, New York, NY, 10276-0910.

    Your certificate will be mailed to you. Normal processing time is 4-6 weeks.
